　　Yes, it's sometimes called an 'incident¹ or accident report form'.

　　It will ask you: How did the accident happen?

　　是让你填写意外如何发生的。

　　What was the cause of the accident?

　　意外发生的原因？

　　Were there any injuries²?

　　是否有伤员？

　　And how could the accident have been avoided³?

1 incident   

n.附带事件，小事件；事件，事变；adj.易发生的；附属的；入射的

参考例句：

• With the passage of time the incident was forgotten.随着时间的消逝，那个事件被忘却了。
• The incident had left him visibly angry and upset.那起事件显然让他既恼火又心烦。

2 injuries   

n.伤害，损害( injury的名词复数 )；受伤处

参考例句：

• His career has been blighted by injuries. 他的事业不断受到伤病的困扰。
• Be sure to wear the correct footwear to prevent injuries to your feet. 一定要穿合适的鞋，以免脚受伤。

3 avoided   

v.避开，避免，预防( avoid的过去式和过去分词 )；[法律]宣布（判决、抗辩等）无效；使无效；废止，撤销
